Group Benefits

For a group quote, the employer or the group’s representative should email Daniel with the following information:

  • your business name
  • business zip code
  • the gender of each eligible employee and any dependents to be insured
  • the date of birth of each eligible employee and any dependents to be insured.

Life Insurance

For a life insurance quote, please email Daniel this information:

  • your name
  • date of birth
  • residence zip code.
